In Outlook 2013 and 2016 there's a "toolbar" above email previews, which contains "Reply" "Reply All" and "Forward". (Mine actually also contains "IM", I think that's because Skype/Lync is installed.)

Is it possible to customise this and add my own items? I can of course add my own pane above it, but would rather appear "integrated", like the "IM" button.

Hello Warren,

Nether the Outlook object model nor the Office Ribbon API provide a way to achieve this. The only way is to show a pane.
