If Outlook opens with multiple Explorers visible on startup, and if your add-in defines a region, the non-primary windows will open with a blank/uninitialised task pane visible with the title "AdxServiceTP".

The hard part is getting Outlook to open with multiple Explorers on startup. I can't cause this to happen consistently, but when working with multiple Explorers and debugging an application, it seems to happen semi-regularly.
