After installing my addin (2003 Server SP1, Office 2003) everything was working great.

After ~2 weeks I've got messages about the application (Word for e.g.) was not terminated properly last time would would you like to open it in safe mode...

The error was about my addin.

Does anyone has any idea?

Hi Ofer.

I think I need a bit more information about the issue.
Do you get any error messages while Word is shutting down?
