xlb preventing showing of the toolbar

My client has an xlb in the Excel\xlstart directory. They reported that the toolbar I created doesn't show up. So this is what I did to recreate the problem.

1) I made sure that my Excel was showing up my toolbar.
2) I copied their xlb to the Excel\xlstart directory.
3) My toolbar doesn't show up
4) Here is the real problem. The toolbar doesn't show up in the Tool | Customize area either so it cannot be added back.

My add in also has a Menu bar that gets added to the File menu. That is not affected by the xlb.

Does anyone have any ideas what could be happening here?

Is your toolbar temporary (Temporary = true)?

I am experiencing the exact same thing. It appears the XLB file is corrupting or something. We have to copy an XLB file from a pc that is working into the affected pc. I have Temporary = False.

I'm not sure, it happens randomly. You have not dealt with this issue before? The tool bar buttons don't load sometimes and we have discovered that by replacing the XLB file with one that we know works it loads the tool bar buttons. We are using Office 2003 Pro and this is happening in Excel.
